Accredited Programs

Annual Report (spring 2012) in Word
        Spring due date is March 15, 2012.
You must submit one complete electronic copy. If the report includes a document that is not available online and is too large to scan, scan only the pages that are applicable to the annual report and attach them to the report. The first page with signatures must have actual signatures and be scanned into the computer for e-mail submission.

Statistics are now page 3 of the annual report, and are due with the annual report.

Annual Statistics are now page 3 of the annual report. Programs on the fall annual report list will now file statistics with their reports in the fall. This requirement must be met even if an annual report is not required because of a recent self-study and on-site.

        Fill out this word document, obtain signatures, and e-mail it to the Director.
